Women's health here runs as continuous care rather than separate appointments: the doctor who confirms a pregnancy is the one at the delivery, and the same clinic handles the years before and after. Do I have to pay to book?

No. Booking is free and there is no online payment. The ₹400 consultation fee is paid at the hospital desk on the day.
