What Dr B. Ravinder does

A cough that has lasted three weeks is the commonest reason people arrive here, and it is worth taking seriously: in this region that list runs from asthma and post-viral irritation to tuberculosis, and the tests that separate them are quick and worth doing early.

Asthma and COPD are the long-term work. Both are managed on lung function measured with a spirometer rather than on how the breathing sounds on the day - and a great deal of the visit is spent on inhaler technique, because a correctly prescribed inhaler taken incorrectly does nothing at all.

Chest X-ray and blood work are in the building and the reports come back the same visit. Breathlessness that turns out to be cardiac rather than respiratory gets walked across to general medicine or cardiology rather than sent away with a letter.

Spirometry
Lung function measured on a machine, so asthma and COPD are diagnosed rather than assumed.
Nebulisation and acute care
Immediate treatment for an asthma or COPD attack, with oxygen if it is needed.
Inhaler technique training
Ten minutes that decide whether the rest of the prescription works at all.
Pleural tap and drainage
Removing fluid from around the lung, both to relieve breathlessness and to test it.
Tuberculosis workup and treatment
Sputum testing, chest imaging and the full supervised course of treatment.
Sleep study referral and management
For snoring with daytime sleepiness, including CPAP setup where it is indicated.
Allergy assessment
Identifying the trigger, because avoiding it beats treating the attack.

  • Bring previous chest X-rays and any sputum reports
  • Note when the cough is worst: night, morning, at work, in a particular season
  • Avoid your reliever inhaler for a few hours before a spirometry test, if you safely can
